Report Identification
This section of the form is used for identification and tracking purposes. The fiscal year and reporting period are automatically filled with the relevant information.

This section is used to identify the organization completing the report and the Recipient who has the reporting requirement with INAC.

- This is a Yes or No answer that causes the PDF to display only those sections that are relevant to you.

- Note: Make sure to enter a 4-digit number. Add zeroes (0) in front if necessary.

List of Reporting Organizations
Use this section to list the organizations that are providing you with their information in separate PDF forms or XML files exported from their SIS.
The fields in this section are:

Contacts
The Primary Contact is the person who is responsible for the DCI when completed.
The Secondary Contact is the back-up contact in case the Primary Contact is unavailable.

School Information
Schools that you do not administer
If you send a student to a school that you do not administer, the only school information you can enter is the school identification and whether there is an agreement with the organization that administers the school.

- This field is automatically populated with the name of the administering organization.

- This field is automatically populated with the type of school administration organization.

Do you have a Tuition Agreement / Letter of Agreement / MOU with the organization that is the administering organization?
- Select Yes or No to indicate whether you have an agreement with the organization that administers the school.
Schools that you administer
If you are the administering organization of a school, you need to update the School Information details.

School Details
This section of the form displays a summary of the school's details that are used to help complete student enrolment information.
The fields in this section are:

In the previous school year, indicate the number of days the school was open for instruction.
- Do not include the number of administration and PD days in the number of instructional days.

In the previous school year, indicate the number of days the school was open for administration and professional development.
- The number of days this school was open for administration and professional development purposes.
Note: The sum of instructional days plus administration days equals the total number of days a school was open.
Student Information by School
Student Information
For registered First Nation students, make sure identifying information is the same as in the Indian Registry System (IRS).
The fields in this section are:
- Family Name, Given Name, Alias Name(s)
- Date of Birth
- Gender

Student Enrolment Status
- If the student attends this school, select Fully (fully enrolled).
- If the student does not attend this school as of September 30 but was there last year, select the reason to explain why.
- Identity
- Indian Registry No.
- Grade
Enrolment Information
This section of the form displays a summary of the student's enrolment details at this school.
The fields in this section are:
- Program
- Select the student’s program of study.
- Method of Delivery
- Select the method that the school uses to deliver programs to the student.
- High Cost Special Education Needs (HCSE)
- Select Yes or No to indicate whether the student has been assessed with HCSE needs.
- Full-time / Part-time
- If the student attends school full time, select 1.
- If the student attends school part time, select the applicable fraction.
- E.g. half time = .5
- Year Graduated
- This field is enabled when the student is in a secondary grade.
Residence
If you do not enter residence information before uploading the EPP Proposal PDF, EIS will generate Errors.

- Select the type of residence or community in which the student lives.

Language Information
Use this section to describe the languages used by the students.
Note: If you do not enter language information before uploading the EPP Proposal PDF, EIS will generate Warnings.

- Extent of Indian Language Instruction
- Indicate how much First Nation language instruction the student receives.

- Select the language primarily used to teach the student.

- Select the language spoken at home.
Accommodation
Indicate if students received living accommodations.

Activity
- Select the type of living accommodation provided to the student.
Transportation
Indicate if students received transportation services.
If student transportation is not required, leave this section blank.

Staff Information
This section is only required for band-operated and federal schools for which you are the Administering Organization.
Note: Education Staff includes those working in both educator and non-certified educator occupations. It does not include information on supply teachers.

- Select 1.0 to indicate full time. If the staff member is part time, select the value that closest represents the proportion of a work week.

- Select the staff member’s self-identified ancestry or heritage.

Provincial Teaching Certification
Use this section of the form to indicate details for the teaching certification of the staff member at the provincial or territorial level.
Note: If the staff member is not certified but is seeking one, select the Status In Progress. Enter the Province and leave the Certification Number blank.

- If applicable, enter the teaching certification number.
Occupations
Use this section of the form to indicate the title or role of the staff member and a percentage of how much of their time is spent in each role.
The fields in this section are:
- Occupation held at the school

Percentage (%) of time
- Indicate the percentage of time for each occupation. This is the percentage of their work day. For example, if the educator is half-time and works ¼ day at an occupation, enter 50%. The sum of all percentages must equal 100%.
Supporting Documents
If you are submitting supporting documents, you need to list them on the PDF form. All documents MUST be submitted electronically.
You can attach any file format such as PDF, XML, MS Word, MS Excel, etc.
