We will use the personal information, provided to AANDC in this Nominal Roll Student and Education Staff Census Report for archival, reporting to senior management and audit, evaluation, research, and/or statistical. The information collected is described in the Elementary and Secondary Education Program Personal Information Bank AANDC PPU 1201 detailed at http://www.aadnc-aandc.gc.ca/eng/1100100011039/1100100011040, will be retained for 30 years and then are transferred to Library and Archives Canada.

As stated in the Privacy Act, you have the right to access your personal information and request changes to incorrect information. Contact your regional AANDC office or the Education Branch at education@aadnc-aandc.gc.ca to notify us about incorrect information. Schools that you do not administer

If you send a student to a school that you do not administer, the only school information you can enter is the school identification and whether there is an agreement with the organization that administers the school.

Note: The sum of instructional days plus administration days equals the total number of days a school was open.

Student Information

For registered First Nation students, make sure identifying information is the same as in the Indian Registry System (IRS).

If you do not enter residence information before uploading the EPP Proposal PDF, EIS will generate Errors.

This section is only required for band-operated and federal schools for which you are the Administering Organization.

Note: Education Staff includes those working in both educator and non-certified educator occupations. It does not include information on supply teachers.

Provincial Teaching Certification

Use this section of the form to indicate details for the teaching certification of the staff member at the provincial or territorial level.

Note: If the staff member is not certified but is seeking one, select the Status In Progress. Enter the Province and leave the Certification Number blank.

Use this section of the form to indicate the title or role of the staff member and a percentage of how much of their time is spent in each role.
