To be included with this completed and signed application:

• Original signed application form, which includes the Designated Representative Consent Form
• Acceptance letter or proof of registration from your school
• Copy of your most recent transcripts
• Copy of current status card (front and back)
• Proof of dependants (if you have any). Example: a National Child Benefit report from the Canadian Revenue Agency, birth certificates.

Reminder: You need to submit transcripts upon completion of each term/semester.
