Indian Registration

Indian Registration (for Bill C-3 Applicants only)

Applicants need to complete and submit the following two forms (Form No. Inter 83-114E and Form No. Inter 83-120E):

If an eligible Guarantor cannot be obtained (Form No. Inter 83-120E), applicants must complete and submit the following form (Form No. Inter 83-119E):

Secure Certificate of Indian Status Application forms (Status Card: for Members of Qalipu Mi'kmaq First Nation Band only)

These forms are intended for members of the Qalipu Mi'kmaq First Nation Band only. All other registered First Nations individuals who wish to apply for a SCIS can find more information on our Secure Certificate of Indian Status webpage.

Secure Certificate of Indian Status Application forms

These forms are intended for all Registered Indians who require a Secure Certificate of Indian Status (SCIS). Please read over and complete the form carefully, because failure to complete all the sections of the form and submit all necessary documentation will result in your application being refused and returned to you. A complete application package allows for improved service through more efficient and timely processing of applications.
